I need help with chemistry as i have difficulty in scoring well and no matter how hard i try to revise the marks i get aren't really what i want (on average i get 58%, but i want at least 80-100% on average) and since i'm in year 12 now i'm really concerned with how i'm going to perform.

Learn the content really really well first
download a bunch of other ppls notes and use it to supplement your own so u have no gaps in understanding

Would it be ideal to write down those notes? Or should i just try reading and memorizing them?

^This.

There is absolutely no substitute for understanding the content.
If you don’t understand something that you read, take the effort to look it up or ask someone until you’re satisfied you understand it. Then you can build on a solid foundation of knowledge & understanding.
