Hi, i want do apply for bonus points because of my out of school sports (i've seen the requirements) and the school advisor said i need to get a form from UAC website, but i can't find any.

Am i meant to go to the uni i want to go to's site? If so, do i need to get a form from each different university and fill it out and give it to teacher?

You're talking about the EAS (Educational Access Scheme)

I don't think this year's form is out yet.
Here is last years application form for you to take a look at until the new one comes out:

Hi, The Elite Athletes or Performers Scheme application form needs to be obtained from the institution that you are wanting to apply for.
