Yes it is possible to transfer.

You will need to apply through UAC (just like you did in Year 12). The opening and closing dates are on the website.

In regards to the requirements, you will need to personally contact the university about this. No one on this forum can give you an accurate answer. Generally, a credit average would get you into most courses.
