well since you said you wanted to study 2 hours every day, put in place systems to force yourself to do so. e.g get a whiteboard or big piece of paper, and write 'DAYS I HAVE STUDIED 2 HOURS' on it and put a mark on it every day you do so so you can keep track.
e.g2 tell your mum not to give you dinner unless you did the 2 hours (you could probably think of something more similar)
e.g3 tell all your family you'll study 2 hours a night, and if you dont they can give you punches or something
e.g4 make a 2 hour study date with a friend (preferably one who wont just talk and dick around) every day. because if make plans alone and dont study you'll only be betraying yourself, whereas if you make an arrangement to study with a friend and dont go, you'll be betraying your friend, which is something you're less likely to do
e.g5 go to the library and borrow a one-week loan magazine, that way you'll be forced to go back to the library one week later to return it or else youll get a fine, and when you're there you'll probably think 'i might as well study'. and then just before you leave, borrow another one-week periodical, then the process repeats..
e.g6 arrogantly tell all your friends that you'll get 99.95 atar guaranteed so you'll be motivated to work hard so you dont look like a complete ass in front of them

dude seems legit... i didnt get the top atar myself, but i came fairly close, so just thought id give abit of advice bro
with your extension maths, learn the concepts from the books, then ONLY focus on doing shitloads of papers, exposing yourself to as many qs as u possibly can, its the best way to prepare yourself for anything they will throw at you, dont waste ur time after uve learnt a concept learning it again in another book, thats not how you do maths
for physics, make sure you not only know the content extremely well (hsc physics is a joke in that they test a fair bit on history society and the environment) but also that you know most of the equations off the top of your head and when to use which one, answer the questions exactly how they want them answered (for the theory questions)
as for your enthusiasm and energy levels, i have a feelin you gone die out soon, calm down a little youre guna burn out before the hsc bro
