for example say for mathematics extention 1, does anyone know how state ranks work? like they pick top 10 but im sure there are more than 10 people who get 100% externals.( assume all state rankers are first internally)

They also use their other maths subject as well to determine it.

so a person who does 3unit but not 4 unit (say if they accelerate 3unit and work hard for only 3unit) is at a disadvantage when attempting to state rank?

Last year a 2u/3u student state ranked ext 1 so they also look at your 2u mark.

Nope, only way is to ask the state rankers yourself. E.G. 94 would land a State rank for IDT and 99/100 would land a state rank for all levels of maths but definitely 100 for extension 1.
