I had a major operation last year and missed out on most of term 4 (start of year 12) because I was still recovering.

My attendance has gotten better during term 1 and term 2 of 2013 but I still take days off when I feel ill and it was only until April when I started doing homework after school. Up until then, all I would do was lie down when I arrived home from school as I was too worn out and had zero energy remaining.

The operation I had takes approximately 1 year or longer to recover and it was for a chronic illness (I still have it but the symptoms have lessened).

Do you think I have a chance with EAS? Should I bother applying?
If I do, could you please explain what the process is like?

I also qualify under this, and to get further information the learning support teacher has been really helpful. I also get special provisions in my actual exams, which you may wish to test your eligibility for as well (again with the disability/learning support teacher).

My school doesn't have a learning support teacher so I don't know who to approach and I am a bit hesitant. Is the school meant to tell parents or are the parents supposed to contact the school about it? I have also been approved disability provisions for my exams too.

Careers Advisor and Year Advisor. The careers advisor will tell you how to apply and who to speak to (My careers advisor taught me a lot about EAS and what I can apply for etc). Your year advisor should know about what has been happening with you and I'm sure will sign or fill out anything required for the EAS form such as the educational impact statement.

Yes you can but you would need to see a psychologist or something for evaluation and comments (any professional usually but best to see someone other than a GP).
