I automatically thought the answer is B, but it turns out the answer is D. I'm 100% certain B is also right, but is it actually wrong?

Would love to know why the answer isn't B.

In multiple choice you are looking for the most correct option. So 2 options can be correct, but 1 is usually deemed as most correct by the markers.

D is the most technically correct/specific.

But D occurs in the application level, it even says it in Samuel Davis' book.

"OSI Presentation Layer – The data is reorganised into a form suitable for subsequent transmission."

iirc it happens at the CCA level.

HSC answers > Sam Davis

It is not impossible for the IPT HSC answers to be wrong, my class found 3-4 wrong questions back in 2014. So i'm not 100% sure.

Checksum wont detect all errors in transmission, so it does not "ensure" this.

It definitely reorganises data for transmission (puts into packets etc) so D is the more correct answer.
