hw will i be eligible for supplimentary exam?

Not all subjects have supplimentary exams and eligibility will depend and is usually decided by the couse co-ordinator.

If you failed a unit and passed it the second time. Does the fail stay permanently on your academic record?

I believe the fail does still stay, yes.

do review of grade and re-assesment both require differrent application forms and what are the chance of passing as i got 46

I had a 49 and it still remained as a fail and had to retake the subject.
