what raw do you need for state rank and what aligned is state rank?

What level of Mathematics are you asking about? Extension 1? General? Extension 2?

A final hsc mark of 99 didn't state rank last year, so yea 100.

For Mathematics in 2013 (Arguably one of the most difficult exam), a raw mark of 99 was tied 7th in the state but other years such as 2012 and 2014 required a raw mark 100 so a 99 can be possible, really depends on the cohort ofc.

Nowadays? What was it before? And when did it start to become 95+?
As in in previous decades, the papers were harder and so 95+% raw wasn't needed to state rank. Iirc there was a paper in the past where the highest mark was like 108 out of 120 or something like that. Now the papers are easier so 95+% raw is generally necessary. Not sure exactly when, but the papers have gradually become easier over the last century (and in particular over the last 20 years or so).

is it possible to state rank with a rank of 3rd but like 1% gap?
