Yes.

You do not need to for aqueous, but I would still recommend to do it.

Yeah fizzy is correct you should! Who knows i doubt it but it maybe could scrape you an extra mark

Correct, balanced equation including solid states.

Thanx fizzy thought so as when i write up equations I am always told to write the states
