Just answered survey. I did mine last year so thought I could help. Don't be offended but I think you should word your questionnaire better. It would be difficult for you to analyse the results if your questions are mainly open questions. Adding closed questions allows you to graph and hence getting a better picture of the sample population you have. Also for ethical purposes you should note that they can remain anonymous if they please. Good luck with your IRP! I know it's a pain in the ass.

Some closed questions would have been nice. However it was nice to vent in that a little bit.
