I hear a lot of people say different stuff, some people skim through all the questions, some people do the MC in their head, some people go straight to the last question so they can keep thinking about it while doing the easier parts.

What do you guys do? And what do you think is the most efficient use of reading time, not that 5 mins matters much but still.

Doesn't matter what maths you do I'd just like to hear your thoughts.

I watch the worried expressions appearing on students' faces.

Just get a general idea of how you intend to approach the paper especially in regards to time restrictions.

Gauge how hard the paper is by skim reading every question, then skip straight to any of the "Harder 3U Questions" looking out especially for probability questions, circle geo and inequalities - since often they have a really nice and elegant method of solving. Look at them, and think of how you can solve them for the rest of reading time. If you really can't, then start looking at multiple choice.
